Free webinar series Thanks to a free webinar series, primary care practices can now benefit from lessons learned in real-world practices that have successfully transitioned to team-based care. The eight-webinar series is brought to you by Community Health Center, Inc. (CHC) and the MacColl Center for Health Care Innovation at Group Health Research Institute. The series is part of CHC’s Clinical Workforce Development Advancing Team-Based Care program, which aims to help health centers learn to collaborate interprofessionally, use data to plan care, evolve nurse care coordination and complex care management, integrate behavioral health, engage their patients, and measure improvement.
